چکیده

The esophagus passes through an opening in the diaphragm (i.e. esophageal hiatus) as it courses through the chest to the abdomen eventually ending at the stomach. This opening is usually adequate for passage of the esophagus and nothing else (Fig. 1). However, patients that have a hiatal hernia have an enlarged opening. There are four different types of hiatal hernias described. Giant hiatal hernia is defined as greater than one third of the stomach in the thoracic cavity1 and representing 5 to 10% of all hiatal hernia.2 The hiatal opening in a patient with a large hernia is wide, with the right and left Crura very thin and often separated by 5 cm or more.2 Types of hiatal hernia are represented diagrammatically in Figures 1 to 2D. Traditionally repair of giant paraesophageal hernia has been performed through laparotomy or thoracotomy, with the advent of laparoscopy, nowadays giant hiatal hernia (type III, type IV) are performed with laparoscopy.3 The recurrence rate after laparoscopic repair of hiatal hernias with paraesophageal involvement (LRHP) is reported to be high.4 Several recent reports have shown laparoscopic repair of paraesophageal hiatal hernia.5-7 Suggesting that it is feasible and effective obtaining comparative result to open surgery. Surgical Repair of Giant Type IV Hiatal Hernia

In this case report, we described a 74-year-old female patient with major complaint of respiratory problemsdyspnea after meal. Chest X-ray of the patient revealed a  gianttype IV hiatal hernia. Preoperative evaluations of the patient included esophagoscopy, esophageal manometry, barium swallow, and CT-scan. Repair of the hernia was performed using a dual mesh on the diaphragm and partial anteri...

The use of autologous fascia lata graft in the laparoscopic reinforcement of large hiatal defect: initial observations of the surgical technique

BACKGROUND Even though there is no consensus, many authors believe that in the cases of large hiatal defects, structurally altered crura and/or absence of peritoneal lining, a crural reinforcement should be performed. Reinforcement could be performed with different techniques and different type of mesh, either synthetic or biologic. The disadvantages of mesh repair include the possibility of se...

Paraesophageal hernia repair.

UNLABELLED Paraesophageal hiatal hernia (PHH), accounting for only 5% of all hiatal hernias, may result in potentially life threatening complications such as obstruction, acute dilatation, perforation, or bleeding of the gastric mucosa. It is traditionally believed that PHH is an indication for surgery. The repair of paraesophageal hernia is technically challenging and controversial. The purpos...

Giant paraesophageal hernia in an elderly woman: laparoscopic fundoplication and mesh in the hiatus

Objective Large hiatal hernias are more common in older patients and they are more likely to be female. In addition, the patients who underwent operation for large hiatal hernia are on average 20 years older than patients without hiatal hernias treated for elective antireflux surgery. The laparoscopic surgery rapidly became the more acceptable approach for gastrooesophageal reflux disease and r...

Laparoscopic hiatal hernia repair. Is the mesh hiatoplasty justified?

AIM Laparoscopic Nissen fundoplication represents the gold standard in GERD therapy, although, a frequent failure of this primary repair is represented by the breakdown of the hiatoplasty. Aim of our work is to evaluate if ultrastructural alterations of the diaphragmatic pillars in patients with hiatal hernia, can explain the physiopathology of hernia recurrence. MATERIAL OF STUDY The patient...
